Description

As a member of the Hospice team, the LPN works primarily in a support role by performing tasks and responsibilities within the framework of case findings, health teaching, health counseling and provision of supportive care under the direction of a registered nurse carrying out the nursing process. The LPN works under the direct supervision of the RN or licensed physician and interacts with patients, spouse, significant others, family members, physicians, and fellow team members in keeping with the mission and mandate of Hospice.

ESSENTIAL FUNCTIONS:

  • Provides services within the boundaries of the State Practice Act and responsibilities listed in the Department of Patient Services.
  • Provides input (including any changes in condition) and supports the decisions and direction of the team as outlined in the hospice plan of care and as directed by the primary RN Case Manager.
  • Make observations and collect data both by interview and physical nature, to identify needs and/or problems and selection of standards.
  • Provides anticipatory guidance and support to the patient/family, according to the interdisciplinary plan of care and as directed by the primary hospice RN Case Manager, Social Worker, Pastoral Care, and/or Coordinator of Patient Care.
  • Record monitored information (i.e., I&O, VS).
  • Participate in patient/family instruction and documentation of instruction and patient understanding on progress notes or teaching protocol.
  • Communicate a daily report on each patient assigned to patient’s case manager.
  • Perform phlebotomy as needed.
  • Perform ADLs when scheduled or needed.
  • Responsible for hourly documentation on a Continuous Care Interdisciplinary Progress Note in the continuous care chart in the home or nursing home. Follows the hospice documentation policy when completing patient documentation.
  • Responsible to notify hospice immediately in the event the patient expires during a continuous care shift. The hospice primary RN or On-Call RN is responsible to attend a hospice death.

  • Can effectively communicate in English, both verbally (in person and by phone) and in writing.


WORK ENVIRONMENT/PHYSICAL DEMANDS:

The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.

  • Work in various home environments and conditions.
  • Various travel to patient homes using reliable transportation with a Valid Driver's License
  • All shift availability with monthly on call weekend rotation
  • Moderate to high-risk of exposure to unsafe environments.
  • Agreeable to work in either county when needed.
  • Ability to carry up to twenty-five pounds of equipment and/or supplies.


PREFERRED EDUCATION & EXPERIENCE:

  • New York State licensed as a practical nurse.
  • Minimum of two years’ experience in nursing – working in homecare or a hospice setting desirable.
  • Attain Hospice & Palliative Licensed Nurse Certification after 500 hours or 1 year of employment.


Hours: Tuesday-Saturday (8:30am-4:30pm)

Pay: $27-29 per hour with Mileage Reimbursement added!

FLSA: Non-Exempt
