Position Description

We have multiple openings for Registered Nurses – Long-Term Care to support the Finger Lakes (NY) Veterans Affairs Healthcare System. These contract opportunities will involve on-site work performed at the Canandaigua and Bath VA Medical Centers.

WORK SCHEDULE: 40 hours/week, evenings and nights available, 8- or 12-hour shifts available (if 12-hours, must work an 8-hour shift every 2 weeks)

KEY TASKS

(1) Provide skilled nursing care to long term care patients with chronic illness or disability that are not capable of caring for themselves for an extended period of time.

(2) Ensure patient-centered and individualized care is provided to promote independence, maximize quality of life, and coordinated to meet the patient’s needs over a period of time.

(3) Deliver safe care and perform nursing procedures to assigned patients

(4) Implement physician, provider, and/or nursing orders in a timely and complete manner.

(5) Complete initial assessment of patient condition and response to treatment and initiate follow-up as needed.

(6) Coordinate and manage long-term care services as part of a multidisciplinary team.

(7) Develop, implement, and revise care plan in conjunction with the patient/family to meet learning needs, and involvement in basic health teaching and discharge planning.

(8) Evaluate and document patient responses to interventions and progress towards attainment of goals and expected outcomes.

(9) Recognize any abnormal signs, symptoms, behaviors, and/or changes in patients' condition and respond appropriately.

KEY REQUIREMENTS

(1) Minimum of bachelor’s degree (master’s preferred) in nursing from program approved by the appropriate state-accrediting agency and accredited by the Commission for Education in Nursing (ACEN) or The commission on Collegiate Nursing Education (CCNE).

(2) Current full, active, and unrestricted license as a Registered Nurse (RN) in at least one (1) state, territory, or commonwealth in the U.S. or the District of Columbia.

(3) Minimum of two (2) years of recent long-term care experience; specific experience working with Veterans preferred

(4) Current Basic Life Support (BLS) and CPR certifications

(5) US Citizenship and ability to complete government-sponsored background check as per contract requirements
