Position Description

Title: Intake RN – Managed Long Term Care (MLTC)

  • Location: Brooklyn, NY
  • Job Type: Full-Time. Onsite.

Compensation:

  • Annual salary: $95,000 - $105,000
  • Bonus: Eligible for a 20,000 sign-on bonus ($5,000 payable every six months, less taxes)

The Health Plans serves over 18,000 frail and disabled individuals, empowering them to live independently in their homes for as long as possible.

As an MLTC-Intake RN, you will assess potential new members for eligibility and explain the program to them and their families. You will play a crucial role in ensuring a seamless transition to managed long-term care.

Key Responsibilities:

  • Conduct outreach activities within the community to assess the eligibility and safety of potential members.
  • Collaborate with the Intake/Outreach team and clinical team to ensure appropriate services are provided to new members.
  • Communicate effectively with potential members, families, referral sources, and physicians.

Qualifications:

  • Current New York State Registered Nurse License.
  • Bachelor’s degree in Nursing required.
  • One (1) year of experience in an acute care setting.
  • Must have UAS-NY experience.
  • Home care and management experience preferred.
  • Proficient in MS Office.
  • Strong oral, written, and interpersonal communication skills.
  • Bilingual preferred
