UAS Assessment Nurse New

HealthCare Support is actively seeking a UAS Assessment Nurse to fill an opening with a large MLTC in White Plains, NY!

UAS Assessment Nurse Duties

• Evaluate patients’ physical, mental, and social conditions through in-person or telehealth interviews.
• Document assessment findings accurately in the UAS-NY system in compliance with state regulations and organizational policies.
• 100% Field I Monday-Friday, 8:30 am - 5:30 pm
  
  
  

Required Qualifications for UAS Assessment Nurse

• BS in Nursing or other health-related area, preferred
• Active and unrestricted New York State RN license.
• 2+ years of experience performing UAS - Uniform Assessment System of New York (UAS-NY)
• 3+ years in MLTC, LTHHCP, MAP, PACE, or other managed care programs
• 5+ years of experience in care/case management, disease management, population health management, utilization review, quality assurance, or discharge planning

Pay Details:

$110,000 - $115,000 + annual bonus
